He asks to buy a bag of syringes. The pharmacy technician walks to grab them from a shelf toward the back.

The man doesn’t have one. I’m sorry, the tech says, I can’t sell them to you without an ID.

He asks again for the syringes. The store’s pharmacist chimes in, repeating what the tech said.

We can lose our license if we sell them to you without an ID, she says, apologetically. Well, can I get someone else who has an ID to buy them for me.

The tech looks apprehensively at the pharmacist. I can’t tell you what to do one way or the other, she says, but we can only sell syringes to someone with an ID.

She says the law is enforced similarly to alcohol sales: secret shoppers from the pharmacy board go to stores and ask to buy syringes. If the pharmacist sells to them without asking for an ID, the pharmacist can be fired, lose their license, the store can be sued — a whole host of bad bad things.

None of that is true.

Additionally, “syringe sales do not require a prescription and pharmacies are free to sell syringes and needles to patients without a prescription,” Campbell explained. Years ago, the pharmacy board and the North Carolina Harm Reduction Coalition put together a pamphlet, which is now published on the pharmacy board’s website.

This is because people who use drugs are at high risk of contracting hepatitis C and HIV if they share or reuse needles. While there are syringe exchange programs and some county health departments that offer new needles and drug use supplies for free, they aren’t located in every community.

Although state law makes syringes available over-the-counter for everyone, there’s nothing in the law that prohibits a pharmacy from setting a stricter policy. NC Health News made calls to various pharmacies — chain and independent — across the state and found that the policies vary widely by store, and sometimes prove oddly difficult to find out.

He also explained Walgreens policy in North Carolina is not to sell syringes to anyone younger than 18. Ingles CFO, Ron Freeman, said the company follows all state and federal rules regarding syringes sales but did not respond to questions about what actual policies its pharmacy staff follow in its North Carolina stores.

Walmart did not respond to questions. Dustin Packer, one of the pharmacists at Drugs America Pharmacy in Robeson County, said the store used to sell syringes over the counter, but since 2017 they’ve required a prescription.

Drug use poses a significant public health burden for the residents of Robeson County. As of December 2021, Robeson had the third highest proportional rate of admission to the emergency department for overdoses, according to data from the North Carolina Department of Health and Human Services.

At Cochrane-Ridenhour Drug Co. in Montgomery County, a pharmacist, who declined to give his name, explained that the store does sell syringes over the counter without restrictions, but requested that information not be publicized.

“We do it in the normal course of our business operations, but I just don’t want people around the county or people who don’t live around here just coming here and just getting syringes. We don’t want to promote that.

“It will probably drive away some of our folks,” he said. Delesha Carpenter, a researcher and professor at the UNC Eshelman School of Pharmacy, studies patient-provider communication as it relates to complicated topics, such as injection drug use.

But despite the discrepancies, in another study that’s currently under review, she and other researchers found that pharmacies were still one of the most common locations where rural people who inject drugs say they get their syringes. “I think pharmacists recognize the public health benefits of syringes,” Carpenter said.

They understand that. “But then they also balance that against [the thought] ‘If I have people who inject drugs coming in, is that going to affect the other customers that are coming in.

’ So as a small business — which a lot of rural pharmacies are — they have a balancing act.”. The concern that if more people who use drugs come into the pharmacy, other people won’t come — while harmful and hurtful to people who use drugs — is not unfounded.

In an October 2021 study, the researchers documented that nearly 60 percent of people surveyed said they would not want someone experiencing substance use disorder to live next door to them, to be their friend, to socialize with them, or to marry into their family. They also found similar rates of stigma among health care workers.

Carpenter has found that emphasizing the public health benefits of non-prescription syringe sales can help get more pharmacists on board. The North Carolina Association of Pharmacists has many videos available on their website to teach pharmacists about harm reduction strategies.

“This idea that we don’t want to rub elbows in the same pharmacy, I just think it’s more a symptom of what’s going on in the broader community,” she said. “Is it an ‘us’ and a ‘them’.

Is there this separation. Or do we view ‘us’ as a whole, and that we’re in this together, trying to get people the health care that they need.

Rural people who use drugs can find themselves especially caught up in the prejudices and diverging policies. According to research from the University of Iowa’s Center for Rural Health Policy Analysis, the number of chain and independent pharmacies in rural areas is in decline.

“In a number of areas, particularly in rural areas, the pharmacist may very well be the only professional health care provider in the area,” said Fred Ullrich, a researcher at the university who co-authored a 2018 study on the decline of rural independent pharmacies nationwide. He and others are in the process of publishing an updated version of the study.

“People can feel free to drop in at their pharmacy and say, ‘I have this problem — you have anything for it. ’” Even though pharmacists can’t prescribe medications, they can point people in the right direction.”.

“There’s over 65,000 pharmacies in the United States, and many of those are in rural areas that are Health Professional Shortage Areas, that don’t have hospitals, that don’t have specialty treatment clinics,” Carpenter said. “The fact that you could go in and see a trained health professional without the need for an appointment, without health insurance, that’s open after hours — that’s just huge in a rural community.”.

“Rural pharmacies are well-positioned to be advocates for harm reduction,” Carpenter said. “But they need to be given the resources.”.

Injection drug use (IDU) accounts for a large number of HIV cases worldwide. Sharing needles, syringes, and drug injection equipment or “works” (including cookers and cotton) allows HIV to be spread from one person to another.

Globally, one out of every ten new HIV transmissions is through injection drug use. However, that average is much higher in some areas.

Blood that contains HIV can also find its way into drug solutions through: People who inject drugs also risk getting hepatitis B (HBV) and C (HCV), both of which are diseases that damage the liver and are spread through blood.

Hepatitis is easily spread through sharing needles and other injection supplies, such as cookers, cotton, and ties. Cleaning your skin before you inject is important.

If possible, wipe down surfaces before injecting. If new needles and injection equipment are not available, cleaning your injection drug equipment with bleach according to the directions below can kill HBV.

The best way to prevent hepatitis is to use a new needle and syringe each time, and never share any part of your works. It is important to get routinely tested for hepatitis A, B, and C.

If available, get vaccinated against hepatitis A and B to prevent infection with these two types of the virus. there is no vaccine to prevent hepatitis C.

Stopping injection drug use altogether is probably the best thing you can do for your health. This may be an incredibly hard thing to do, and it may not work for everyone.

Drug treatment programs are available in many countries to help you stop injection drug use. Some programs have waiting lists and women with children may need to make special arrangements.

You might not think of yourself as having “shared” a needle and syringe if both you and a close friend or acquaintance used them. But sharing needles and syringes with friends can be as dangerous as sharing with strangers.

Do not assume a needle and syringe are new, even if they seem to be packaged as new. Get needles and syringes from reliable sources, such as pharmacies.

If you cannot buy new needles and syringes from a pharmacy, look for a needle exchange program (also called a syringe exchange program or syringe service program). Needle exchange programs can give new syringes and needles to people who inject drugs to prevent the spread of HIV and other diseases that are transmitted through blood.

The WHO strongly supports the use of evidence-based needle and syringe programs as part of HIV prevention. There are needle exchange programs in some countries, but they are not available everywhere.

Needle exchange programs offer a good way for people who inject drugs to reduce the risk of HIV transmission. In addition, these programs may be able to help you get benefits, medical care, and access to drug treatment programs.

If you must share needles and syringes because new, unused ones are not available, always clean the needles and syringes with bleach and water immediately after use and just before using them again. Keep in mind that cleaning with bleach does not make re-using the equipment risk-free.

To be effective, you must carefully follow the cleaning procedures listed below every time. Pour clean water into a cup, cap or something that only you will use.

Fill the syringe by drawing the water up through the needle to the top of the syringe.

Squirt out the water and repeat at least three times (do not reuse water).

Fill the syringe by drawing the bleach up through the needle to the top of the syringe.

Leave the bleach in the syringe for at least 30 seconds.

Pour new clean water into a cup, cap or something that only you will use. Don’t use the water from part one.

Fill the syringe with water, to rinse out the bleach. Fill the syringe by drawing the water up through the needle to the top of the syringe.

Shake it around and tap it for at least 30 seconds.

Some tips: Sharing a needle, syringe, or any related equipment for any use, including skin popping, tattooing, body piercing, or injecting steroids or hormones, can put you at risk for acquiring HIV or other blood-borne infections.

The safest thing to do is to stop injecting drugs altogether. If this is not possible right now, get new, sterile needles each time you shoot up or find a needle exchange program.

If you are at risk because you are injecting drugs now or have injected in the past, get tested for HIV and hepatitis. You can also put your sexual partners at risk if you have unprotected sex, so it is important to use condoms and practice safer sex.

Over the past decade, attempts to address the overdose crisis in the U.S. have resulted in more restrictive opioid prescribing policies—which, because they have reduced the overall availability of prescription opioids, have inadvertently led to a surge in the use of illicit drugs such as heroin.

Because these illicit opioids are often injected, they’re associated with higher rates of overdose than prescription painkillers, and, when unsterile injection equipment is used or shared, people are at risk of getting a serious infection or transmitting disease.1 As of March 2020, the Centers for Disease Control and Prevention determined that 44 states.

Such programs also often offer other services, such as vaccinations and education on preventing overdoses, and have also been shown to improve the odds that PWID will seek and engage with treatment. Contrary to community concerns that SSPs encourage drug use or criminal activity, more than three decades of peer-reviewed research has instead shown them to reduce rates of HIV and HCV among PWID.

Recent evidence from Miami, for example, suggests that implementation of these programs also helps to reduce overdose-related hospital admissions, and that SSP costs are minor compared with the cost of treating HIV infections—which SSPs can help prevent.6 Given the public health benefits of SSPs, the U.S.

However, local jurisdictions hoping to establish and operate SSPs face many state policy barriers. In addition, cost, stigma, and misconceptions about these programs and their participants have prevented some states from taking action to support the implementation of SSPs.

State legislatures typically must authorize SSP operation in order to exempt programs from laws that criminalize drug paraphernalia, such as the syringes these programs distribute.

The following steps would help to ensure the successful establishment and operation of SSPs: Syringe services programs (SSPs), also commonly referred to as syringe or needle exchange programs or harm reduction programs, were developed by and for people who use drugs to reduce the harms of injection drug use by providing access to and disposal of drug use equipment and other related services.

Sterile supplies distributed for injection drug use include syringes, cookers, cotton, tourniquets, and clean water.

Although 31 states and the District of Columbia legally authorize SSPs, some have ambiguously worded laws that make implementation and expansion of SSPs difficult—and severely limit the number of people they can reach.14 For instance, nine of the 32 also require local approval for SSP implementation, which is a challenge because SSPs often face community opposition due to stigma against PWID.

To maintain the supply of syringes and effectiveness of SSPs, state legislation should prevent localities from limiting syringes and focus on evidence-based approaches. North Carolina’s law does just that, explicitly stating that sterile injection equipment be supplied “in quantities sufficient to ensure that [they] are not shared or reused,” which effectively assures their unlimited distribution.25.

State drug paraphernalia laws, which typically prohibit possession of syringes for the purpose of illicit drug use, are often at odds with SSP objectives.

The Baker Institute Drug Policy Program began in 2001. After an inaugural conference in April 2002, the first topic I chose to write about was needle exchange programs, a proven way to reduce the transmission of blood-borne diseases such as HIV/AIDS and hepatitis among injecting drug users.

I was mistaken. In this issue brief, I will summarize the scientific and fiscal evidence supporting needle exchange, trace recent developments, and recommend a framework for dealing with these and other aspects of drug use and abuse.

In recent years, needle exchange programs (NEPs) began to be called Syringe Exchange Programs (SEPs), properly reflecting the fact that HIV and hepatitis viruses could live not only on the needle, but also within the syringe itself.

In this publication, I will use the newer labels. A contaminated needle is an extremely efficient transmitter of blood-borne disease, and the virus can live on a needle or within a syringe for as long as four days.

The illegal drugs most commonly injected are heroin, cocaine, and methamphetamines, although each of these can be taken by other means, such as snorting, smoking, or ingesting orally. Prescription drugs can also be crushed into powder and made injectable.

This inherent risk is substantially increased when users share needles contaminated by blood-borne diseases, most notably HIV/AIDS and hepatitis A, B, and C. The actual result is stunning.

By the end of 2002, according to the CDC, 32 percent of people living with AIDS in the United States were PWID, as were 28 percent of new diagnoses in that year.2. By 2015, the proportion of new HIV/ AIDS diagnoses had dropped to 9 percent and the CDC credited SSPs with playing a significant role in that decline.3 Hepatitis C, the most dangerous variant of that disease, is also rampant among PWID in the United States.

The lifetime cost of treating either of these diseases can easily exceed $300,000, much of which is paid by Medicaid, Medicare, local hospital districts, the U.S. Department of Veterans Affairs, and other public funds.4 Thus, every case of HIV/ AIDS or HCV prevented by syringe exchange carries an extraordinary cost benefit.

At least eight major studies, conducted by such organizations as the National Academy of Sciences, the CDC, the American Medical Association, and the World Health Organization, have unanimously concluded that access to clean needles dramatically reduces the incidence of blood-borne diseases and neither encourages people to start injecting drugs nor increases drug use by those who are already users.

Syringe exchange is now an accepted part of public health programs in almost all countries of Western and Eastern Europe, Central Asia, and Australia and New Zealand. In some locales, sterile syringes can be exchanged at pharmacies, police stations, and even from specially designed vending machines.

Persuaded by scientific consensus and international examples, numerous U.S. cities have established SEPs that produce positive results.

Many are understaffed, underfunded, and unable to offer anything like the services that are needed. But when run well, they can dramatically reduce the spread of diseases.

After the Baltimore City Needle Exchange, established in 1992, had been in operation for six years, the incidence of HIV in that city dropped by 35 percent overall and 70 percent among the approximately 10,000 participants in the program.

Thanks to SSPs and other prevention measures, it had fallen to 13 percent by 2001 and is now down to less than 1 percent. Epidemiologist Don Des Jarlais, director of research at New York’s Mount Sinai Beth Israel medical center and a leading expert on syringe exchange, said, “We appear to be very close to eliminating injecting-related transmission in a city with over 150,000 injecting drug users.”7.

The map in Figure 1, prepared by the CDC in October 2016, shows (in pink) U.S. counties deemed to be particularly “vulnerable to rapid spread of HIV and hepatitis C infection among people who inject drugs.” Of 220 such counties in the nation, the most by far are concentrated in central Appalachia (Tennessee, Kentucky, West Virginia, southern Ohio, and Indiana).

In addition to having four-fold increases in hepatitis C infections and comparable increases in HIV, these counties are also marked by “high unemployment and poverty, low educational attainment, and limited access to health care,” all factors known to contribute to drug abuse.8. As their dependence increased and as reformulation of the drugs and restrictions on prescribing them made them harder to obtain, many users devised ways to crush pills into injectable form, which provided a quicker and more intense rush than ingesting them orally. Many turned to heroin, soon more readily available and cheaper, but also with no pharmaceutical label to indicate its strength.

In the absence of SSPs or other ways to obtain sterile syringes legally, PWID shared those they could obtain, sharing disease in the process. As the opioid epidemic progressed, each state has been forced to confront its stubborn resistance to syringe exchange and to make some accommodation to settled science.

After several years of an average five new diagnoses of HIV per year in the southeastern part of Indiana, 169 new cases appeared by mid-June 2015 and more than 20 new cases were being diagnosed every week. More than 80 percent of those also had HCV.10 The epicenter of the outbreak was Scott County, on the border with Kentucky, just north of Louisville.

They also had little opportunity to learn whether they were infected after spending cuts and thenGovernor Mike Pence’s opposition closed the Planned Parenthood office that had been the only HIV testing center in the county. As a result, drug users spread the virus by sharing needles.

Although by law in many states, pharmacies are allowed to sell syringes over the counter, the pharmacist may still ask you about the syringe you’re using to find out the purpose. Pharmacists solely do this to avoid inadvertently catering to drug abuse.

It is important to note here that though many states do not require a prescription, a couple do require a prescription to purchase any amount of insulin syringes. Other states may ask that you have a prescription if you are buying a pack of more than 10 syringes.

If you only have a rough idea about the syringe you want to buy or if it’s the first time you’ve asked, then it’s always good to just be open about your diabetes condition.

On the contrary, it could help them provide a better recommendation than the one you had in mind.

This also gives you a good time to explore the varieties of syringes available to best suit your needs and get information directly from the pharmacist as opposed to reading or researching everything about it online. If you have an idea about syringes and how they work, then you can mention your preferences to the pharmacist.

You can always discuss your preferences before asking for any recommendations. Stating your preferences also makes it easier and timesaving for the pharmacist to provide or recommend the most suitable one for you.

Mentioning drawbacks with past syringes are important for making a feasible investment in the next set of syringes. This will prevent the pharmacist from offering similar syringes like the ones you might have had before and perhaps had trouble with.

If you’re thoroughly aware of the ins and outs of the syringes you need, then make sure you specify and give details, so that the pharmacist can provide the exact syringe you need. This includes asking about what quantity you’re allowed to obtain on this visit.

But of course, this takes a lot of experience in buying, using, and getting to know about syringes thoroughly. You can also get recommendations by asking your doctor, health professional, or others who manage diabetes themselves.

The gauge of the syringe needle specifies the thickness or width. Syringes range from 28 gauge to 31 gauge.

For children, the smaller gauge needle is often said to work best. In contrast, older people and large children may find a larger needle better.

The most common needle lengthsare 12.7 mm (1/2″) and 8 mm (5/16″). The 8-mm needle is called “short,” and is the needle length that most diabetes patients prefer.

Choosing a barrel size should be matched with your insulin dosage. A 3/10-cc syringe works best for holding 30 units or less.

On the higher end, a 1-cc syringe works best for 50-100 units.

For example, “I’m looking for 100 insulin syringes, 1/2 cc, 28 gauge needle, 5/8 inch.”. If you’re unsure, then talk through your needs with the pharmacist, and they may be able to make a recommendation for you.
